Creative Services Forms

Please note that all project requests are subject to review before projects are accepted. Projects are reviewed for alignment with institutional strategic goals, the schedule and workload of OCM team members, and the timeline needed to complete the proposed work.

Deadlines: All due dates provided on forms and by Print Shop Pro are estimates until confirmed by the OCM Administrative Coordinator. Print Shop estimates are based on the date of approval for graphic orders. Delays in approval for graphic orders on behalf of the client department may result in delays in production. All project due dates are subject to the timing of contract and procurement approvals as necessary from Financial Services.
